Can my EPS require me to comply with a minimum contribution period in order to access treatments or procedures included in the POS that are considered high cost?

In the case of HIV/AIDS, as it is classified as a catastrophic illness, the rules regarding minimum contribution periods must be applied when this type of medical care is required and it is demonstrated that the cost cannot be covered.
